Job Summary

We are seeking a dedicated Registered Nurse (RN) or Licensed Practical Nurse (LPN) to join our nursing leadership team in a dual-role position. Depending on licensure, this role will function as either an Assistant Director of Nursing (ADON)/Treatment Nurse (RN) or Quality Assurance Nurse (QA Nurse)/Treatment Nurse (LPN). This position plays a vital role in supporting clinical leadership, quality assurance, wound and treatment services, and daily nursing operations. This role requires participation in an on-call rotation, including weekends, to ensure continuity of care and leadership support.

Essential Job Functions

Provide leadership support to nursing staff and ensure compliance with facility policies, state, and federal regulations.

Perform and oversee daily treatment orders, including wound care, skin integrity, respiratory, circulatory, and other ordered treatments; promptly report changes in resident condition or wound status to the physician.

Complete and maintain weekly skin reports, including resident response to treatment, wound descriptions, and assessments, and monitor Braden Scale scores to ensure appropriate skin intervention protocols are implemented.

Collaborate with the interdisciplinary team to develop, implement, and update resident care plans; attend required meetings including S.W.I.F.T., care conferences, quality, and interdisciplinary meetings as assigned.

Work closely with nursing staff to implement and monitor programs aimed at preventing preventable skin breakdown and promoting resident safety and comfort.

Perform resident rounds to ensure care plans are followed and residents' needs are addressed; assist with writing and updating resident assessments, care plans, MDS documentation, and Quality Indicators as required.

Perform all required audits

Accurately receive, transcribe, and implement physician orders (verbal or written) on MAR/TAR, notify the Unit Clerk of appointments, and ensure the overall accuracy of clinical work.

Maintain complete, accurate, and timely documentation, including but not limited to:

MAR/TAR documentation for medications, treatments, and supplements administered or not administered

Daily nursing documentation, Medicare and MDS charting, charting by exception, and diagnostic/lab result documentation

Completion of all assigned MDS assessments

Participate in quality assurance activities, audits, infection control initiatives, and performance improvement programs.

Assist with staff education, training, scheduling, and performance oversight.

Provide floor coverage as needed

o When assigned to floor coverage, oversee nursing care on the assigned wing, including medication accountability, CNA assignment completion and delegation within scope of practice, and carrying out physician orders related to medications, treatments, labs, and diagnostics.

Directly reports to and works with the Director of Nursing.

Participate in care conferences, interdisciplinary meetings, and survey preparedness.

Provide on-call coverage, including weekends, as scheduled by administration.

Perform physical requirements of the role safely.

Working Conditions/Physical Requirements

Ability to lift up to 100 pounds and assist with resident mobility.

Frequent standing, walking, bending, and lifting.

Exposure to healthcare environments and safety-sensitive conditions.

Participation in on-call rotation, including weekends.

This is a safety-sensitive position pursuant to A.R.S. 23-493.

Required Minimum Education, Experience, and Training

Graduate of a state-approved school of nursing.

Current Arizona RN or LPN license or compact license.

CPR certification.

Arizona DPS Fingerprint Clearance Card.

Completion of annual in-service training and health requirements.
