Job Overview
Prototypes S. Mark Taper Family Living Center is a 32-unit apartment complex that shares a campus with Prototypes Residential Substance Abuse Treatment Program and Prototypes Outpatient Program. The Housing Program at S. Mark Taper Family Living Center serves women and women with children with Substance Abuse and Mental Health Disorders. The Housing Program also services 37 scattered-site clients living around the Pomona area. The Housing Program provides case management services to help clients work toward acquiring permanent housing. Prototypes Housing Program is a fast-paced environment, as we strive to remain on the cutting edge of service provision.
The RBH Case Manager I/II is a registered or certified candidate who maintains registration/certification with an approved/accredited California agency. The RBH Case Manager I/II is responsible for delivering services to a caseload of RBH Housing clients and providing Individualized Substance Use Disorder case management services. The RBH Case Manager I/II works in collaboration with other members of the multidisciplinary team to ensure best possible treatment outcome for the clients and provides treatment in accordance with Healthright360's philosophies, goals, policies, mission, and vision. The RBH Case Manager I/II may provide treatment utilizing different modalities including individual and/or group settings.
Key Responsibilities
• Assists clients with intake, assessment, and orientation to program upon admission or transition in care-level.
• Facilitates Substance Use Disorder groups
• Provides facility coverage for Housing Program
• Monitors clients' progress toward achieving treatment plan goals and provides treatment plan input
• Utilizes Trauma-Informed Care while applying Motivational Interviewing, Cognitive Behavioral Therapy, and Evidence Based Practices
• Facilitates Weekly House Meetings
• Seeks clinical consultation throughout the treatment episode and when encountering new and/or high-risk clinical circumstances
• Maintains frequent communication with the treatment team and engages in regular consults, clinical teams, case conferences and plan/development meetings
• Delivers services in Outpatient, Residential, or field, depending on assignments and client needs
• Responsible for treatment oversight of the clients on their caseload
• Assists with client program oversight by managing client business passes, privileges, check-in, and treatment compliance
• Meets expected client care hours (productivity expectation is 25 client care hours per week
Education and Knowledge, Skills and Abilities
Experience:
Preferably, a minimum of 2 years experience in the Human Services Field and demonstrated expertise in Substance Abuse Treatment, Relapse Prevention, and recovery
Experience working with clients experiencing acute withdrawal from substances
Knowledge of and experience providing Trauma Informed services
Experience delivering Evidence Based Practices
Education:
Maintains registration/certification from an approved/accredited California agency Background Clearance.
Live Scan clearance required
Must not be on parole or probation
