JOB SUMMARY
Our services at the Ontario One-Stop TAY Center are delivered in a comprehensive manner. We meet the client where they are at and strive to guide clients towards successful outcomes. We have a program that is designed to take care of the basic needs of the consumer (food, shower, laundry) while providing a more in-depth experience that involves counseling (therapy), case management, and psychiatric services. We have partnered with the County of San Bernardino to provide such a unique service throughout San Bernardino County. JOB SUMMARY
The Case Manager/Psych Tech assists with identifying and coordinating community services to assist and support clients with mental health and substance use disorders. Recommends and assists with access to medical, educational, social, prevocational, vocational, or other community services to support treatment and recovery goals. Also fills in for the Psychiatric Technician, providing care and assistance, to assessment of general physical condition and assistance with medically prescribed prescriptions, to the mentally ill, emotionally disturbed or mentally challenged clients participating in relevant treatment activities and programs.
RESPONSIBILITIES

Conduct intake interviews and complete individual assessments of client status and level of comprehensive services required. Prepare progress notes and other records and documentation.
Make appropriate community services referrals, coordinate and arrange for treatment placements, and arrange and transport clients for pre-placement visits.
Conduct chart and treatment plan reviews to ensure compliance with government, agency and contract standards.
Prepare critical care issues for supervision discussion and weekly case presentations for admission review.
Maintain a caseload. Maintain contact with treatment providers to continuously appraise client progress.
Provide crisis intervention.
Communicate with and provide support to client and family members and encourage family members to participate in client stabilization and counseling services.
Serve as a backup Psychiatric Technician as needed, providing services for the mentally and emotionally disturbed clients that include observing and evaluating behavior, recognizing symptoms, taking vital signs, and assisting with hygiene and nutritional needs.
Transport clients when necessary.
Comply with all MHS and program rules, policies and procedures including HIPAA standards.
QUALIFICATIONS
Bachelor's Degree or Associates degree preferred, High School/GED required
Valid California Psychiatric Technician License required, issued by the California Board of Vocational Nurse and Psychiatric Technician Examiners.
Minimum of two years providing substance abuse treatment and care to adults
Demonstrated in-depth knowledge of substance abuse treatment methodologies and diagnostic techniques.
Ability to communicate verbally and in writing
Proficient with Microsoft Office Suite and EHR software
Ability to problem solve and react appropriately to emergency situations
Ability to work independently and effectively collaborate with a team
Valid California driver's license and current auto insurance coverage
