Who we are:
Child & Family Healing Center provides 24- hour intensive residential treatment on UMFS's staff-secure, unlocked 33-acre campus. Our multidisciplinary team integrates Collaborative Problem Solving with family engagement, psychiatry, nursing, verbal and non-verbal therapies, social work, and counseling.
What we do:
We collaborate with communities in a Systems of Care approach to ensure successful transition into residential treatment, through intervention, and back into the community.
We assess strengths and executive functioning to develop treatment goals for youth and families.
Who We Serve:
We serve males, females, and gender-diverse youth ages 11-17 at time of admission, who are in the custody of their parent/guardian, the Department of Social Services, or the Department of Juvenile Justice, and have a minimum IQ of 65 (evaluated on a case-by-case basis).

What we are looking for:
A Clinical Case Manager to provide comprehensive case management and care coordination to children and families at CFHC. This includes collaboration with caregivers and legal guardians as well as external stakeholders including locality case managers, CSA/FAPT members, DJJ representatives, UMFS programs, and community providers.
This role would support the caregiver and legal guardian from admission to discharge in regards to moving towards successful discharge to a lower level of care and addressing concerns and barriers to discharge in a collaborative manner.
What you will need:
Master's degree in social work, counseling, or other human service degree, LCSW or LCP Certified preferred or Bachelor's degree in these fields with five or more years of experiences in the Human Service field.
