About the Role:
The Clinical Dietitian coordinates and implements the nutritional care of patients suitable for alternate site care, to include Home Infusion and the Ambulatory Infusion Clinic (AIC) setting, with a primary focus on patients requiring parenteral nutrition therapy.
Schedule: Monday - Friday; 8:00 AM - 5:00 PM, Saturdays as needed.
This position may float to other areas as needed.
Minimum Qualifications:
Experience/Education:
High School Diploma or Equivalent.
Bachelor's degree in Nutrition, Dietetics or related field from an Academy of Dietetics (AND) accredited institution and/or approved internship program.
Registered Dietitian.
Certified Nutrition Support Clinician (CNSC).
5 years of Registered Dietitian experience, with minimum of 3 years of parenteral nutrition therapy experience.
Proficient at partner programs (i.e.: MS Outlook, Word, Excel).
Ability to cope with ambiguous, changing environment while under pressure in a calm manner.
Must be able to work as a team member and develop productive and cooperative working relationships with all members of the Palmetto Infusion/AccuRX team.
Essential Functions:
Performs nutritional assessments, makes recommendations for nutritional therapies, establishes nutrition care plans, and monitors the effectiveness of nutritional therapy.
Supports the Intake Department by assisting with payer qualification for parenteral nutrition patients.
Communicates effectively with clinical, reimbursement, operational, and management personnel as required to coordinate home infusion referrals.
Assists in parenteral nutrition order adjustments to maintain appropriate nutrition plan of care for patients.
Develops and provides nutrition and therapy-specific education programs and materials to patients, Providers, caregivers, and organizational staff, designed to improve health and outcomes.
Supports sales by creating and conducting marketing initiatives to promote the Clinical Nutrition Program and service offering.
Provides nutritional care to all ages of patients from newborns to the elderly.
Works collaboratively with a multi-disciplinary Health Care Team to promote outcomes through evidence-based practices.
Maintains clinical documentation in accordance with best practice and accreditation standards.
Completes and submits all required patient care documentation and reports in an accurate and timely manner.
Maintains knowledge and proficiency in evidence-based best practices through continuing education, staff meetings, and in-service opportunities.
Works collaboratively with intercompany departments and leadership to establish nutritional program protocols, policies, and procedures.
Participates in the orientation of other staff, as requested.
Takes on-call responsibility as assigned to provide 24-hour availability according to protocols.
