The Compliance Facility Oversight Liaison is an integral part of our service delivery and compliance processes forInnovAge PACE/LIFE contracted facility providers (i.e., long-term care, skilled nursing facilities, assisted living andboarding facilities). They partner to identify new program partners and practice routine compliance audits of our existingpartners. They review our partners to ensure they are in a safe place for our InnovAge participants ensuring all services andsafety practices are reviewed. It is a delicate balance to review our partner's living conditions and activities monitoring areasof improvement for deficiency to be monitored versus reported and managed through to a corrective action. They must havea clinical lean on evaluating the practices to ensure living conditions, wound care and mediation administration are theInnovAge standard such that we mitigate risk to InnovAge PACE/LIFE participants residing at our contracted facilities.

Essential Functions and Work Responsibilities

Functional Category:

Compliance and Risk Management of Contracted Facilities

Estimated Percent of time Spent - 100%

Assists with facility network expansion activities for InnovAge PACE/LIFE by performing initial vetting of newfacility contract requests. Providers support to InnovAge PACE/LIFE operations to ensure compliance with partnershipagreement.
Proactively gathers information and monitors survey results for contracted housing facilities to identify adverse results.
In a collaborative and partnership like style performs audits of documentation, departments and function of contractedfacility providers to ensure adherence to regulatory guidance. Presents findings and recommendations to leadership.
Recommends corrective action to reduce regulatory non-compliance. Monitors the effectiveness of corrective actionand coordinates follow up as required. Assists with policy and procedure development and maintenance.
Recommends the suspension of admissions and termination of contract to management and leadership as necessary.
Recommends and implements corrective action in order to improve quality of care and to increase collaboration andcontractual compliance in housing providers.
Facilitates resolution of care, quality or collaboration issues in contracted facilities and ensures special case monitoringand action planning to increase quality compliance and mitigate risk.
Proactively works with PACE/LIFE operations staff and leadership to ensure effective partnership with contractedfacility providers and in identifying new partnership needs.
Works with InnovAge Legal Department to help enforce contractual obligations in the event of suspension ofadmissions or termination of contracts with housing providers.
Builds and maintains relationships with community agencies such as the PACE Ombudsman Program, Long TermCare Ombudsman Programs and Health Department in order to proactively manage emerging risk issues in InnovAgecontracted facilities
In collaboration with the Operations and the Education Department, designs and develops traininginitiatives/interventions to address non-compliance.
Supports regulatory audits for all PACE programs, CMS, State Administering Agencies and State licensing agencies. Assists with mock audits.
Performs other related duties as assigned.

Travel

Travel between InnovAge worksites
Some overnight travel out of state

Work Experience/Qualifications/Education

Associate degree or higher in a Healthcare related field from an accredited college/university. A current state issued RN license is preferred.
Minimum of three years working in/with a long-term care or assisted living facility and minimum of one year workingwith the frail or elderly is required.
Current and relevant knowledge of applicable state and federal regulations for long-term and assisted living facilities.
Preferred prior experience working in a PACE facility.
Knowledge of interviewing and investigation techniques and implementing corrective action plans.
Experience providing services to the elderly in addition to practices and techniques related to quality elderly care.
Skilled at having difficult and direct conversations, identifying problems, rapport and relationship building, analyzingand assessing situations.
Strong negotiation and project leadership skills.
Communicates effectively both orally and in writing. Able to act independently and assertively. Interprets and explainscomplex rules and regulations.
Bi-lingual (Spanish) preferred.
Requires valid state issues driver's license, personal transportation and good driving record and auto insurance as required by law.

The pay offered for the position will take into consideration the candidate's geographic region, job-related knowledge, skills,experience, and internal equity, among other factors.
