Description
  • Conduct thorough evaluations of patients in either an inpatient or outpatient context and develop personalized treatment plans.
  • Implement physical therapy interventions centered around enhancing mobility, strength, balance, and overall function.
  • Collaborate effectively with nurses, physicians, case managers, and other rehabilitation professionals to maximize patient outcomes.
  • Educate patients and their families about therapeutic objectives, discharge strategies, and at-home exercise regimens.
  • Maintain clear and precise documentation of evaluations, treatments, and patient progress in alignment with both facility standards and regulatory requirements.
  • Engage in care team discussions and interdisciplinary meetings to ensure comprehensive and coordinated care for all patients involved.
Education
  • Doctorate in Physical Therapy (DPT) or a relevant degree from a reputable accredited institution.
Certifications
  • Valid state licensure as a Physical Therapist.
  • CPR certification is preferred but not required.
  • Any specialized certifications (e.g., neurological, geriatric) are advantageous, though not mandatory.
Skills
  • Strongly preferred previous experience in a clinical setting.
