We are looking for a Medical Social Worker (MSW) to support hospice patients at home. In this role you will collaborate with members of our interdisciplinary teams to ensure that your clients' non-medical needs are met and that they have the supports that they need in place.

This role is eligible for a $2000 sign-on bonus

Primary Job Responsibilities

Working in hospice, you will have the unique opportunity to provide direct, one-on-one supports to individuals in a setting where they are most comfortable, at home
Develop trusting, meaningful relationships with your clients over extended periods of time - something that is not possible in a hospital or facility setting
Develop, communicate, and coordinate a goal-oriented plan of care in collaboration with the patient, their medical doctor, and family. This includes care plans for LNAs as needed
Maintain consistent, timely, and accurate communication with all members of the patient's health care team
Keep up to date electronic documentation of care provided
Evaluate each person's progress in meeting their goals of care and update plan of care accordingly
Evaluate evidence-based treatments for clients with psychosocial challenges that interfere in their ability to abide by medical plan of care

Job Requirements

Master's degree in social work or BSW
Minimum one year of progressive clinical experience in medically-oriented setting.
Exceptional new graduates will be considered
