$90 - $110 per assessment

Overview

Our client is seeking an experienced Nurse Practitioner or Physician Assistant to perform in-person, no-cost annual preventative health risk assessments for Medicare, Medicaid, and Affordable Care Act members. The provider will conduct visits within members' homes and must be comfortable working independently, traveling within a defined geographic radius, and maintaining high-quality clinical documentation.

This per diem role offers flexible scheduling and requires a minimum of 15 completed assessment appointments per month.

Key Responsibilities
  • Perform in-person annual preventative health risk assessments in members' homes
  • Successfully complete required self-paced and instructor-led training
  • Provide a minimum of 15 appointment slots per month
  • Ensure high-quality assessments that meet all satisfaction and quality expectations
  • Document and submit chart notes within 24 hours of each visit
  • Respond to charting queries within 48 hours
  • Maintain effective communication with clinical leadership
  • Attend periodic meetings and training sessions
  • Safely enter and work within members' personal living spaces
  • Lift up to 20 lbs as needed
  • Bend or kneel for extended periods when required during assessments
Required Qualifications
  • Master's or Doctorate degree from an accredited NP program (FNP, AGNP, ANP)
  • OR Master's degree from an accredited PA program
  • Home health experience within the last 5 years
  • Strong computer skills and proficiency with electronic medical records
  • Understanding of CMS and ACA risk adjustment models
  • Ability to document assessment findings accurately and thoroughly
  • Strong verbal and written communication skills
  • Ability to maintain HIPAA compliance and patient confidentiality
  • Ability to make sound clinical decisions within established guidelines
  • Ability to adapt to changing work conditions and responsibilities
  • Ability to anticipate and resolve member, provider, and health plan issues
Preferred Qualifications
  • At least 1 year of in-home assessment experience (highly preferred)
Certifications/Licensure
  • Current RN and NP or PA licensure in the state of practice
  • Board certification (ANCC, AANP, or NCCPA)
  • State Medicaid identification number
  • Medicare PTAN number
  • Current BLS certification
  • Valid, unencumbered driver's license
  • Proof of adequate auto insurance coverage
