UnityPoint Health - Eyerly Ball is seeking a full-time Jail Diversion Case Manager in the Jail Diversion Department serving Polk County.
The Jail Diversion Community Case Manager position requires knowledge of assisting clients in the areas of services referrals while collaborating with other resources to secure mental health services for jail inmates, and ensuring inmates begin utilizing assigned services when released from jail.
In the role of Jail Diversion Case Manager, qualified candidates will be responsible for ensuring that quality services are provided on a timely basis at the request of the County Mental Health Services and law enforcement personnel.
As a Jail Diversion Case Manager you will:
Determines appropriate provider of services, assists with the development of a client crisis plan.
Coordinates referrals for pre-commitment screenings. Works closely with concerned others supporting the individual seeking mental health services.
Advocate for clients to help meet their needs, desires and best interests.
Responds to calls from law enforcement personnel related to mental health issues.
Meet with incarcerated individuals at county jails at request of law enforcement personnel.
Utilize critical thinking in making independent judgements and maintain responsibility and accountability to our clients.
Capacity to professionally relate to a wide variety of clients, as well as a desire to serve as a client advocate.
Ability to practice good verbal and written communication skills with ability to complete case notes timely.
Capacity to communicate with clients effectively, patiently and respond professionally at all times.
Ongoing passion about making a difference in the lives of those who have been touched by mental illness.
To be considered you'll need:
Bachelor's degree in human services or related field
Two years' experience in human services field, preferably in a jail or re-entry capacity
Extensive knowledge of community based services available to address client needs
Possess a valid driver's license
Required to provide proof of auto insurance coverage
Monday-Friday, 8:00 am to 4:30 pm
