Duties and Responsibilities:

• Deliver skilled physical therapy services in accordance with physician orders.
• Evaluate patient needs, create and adjust care plans to achieve therapy goals.
• Collaborate with interdisciplinary teams to set and review goals for each patient.

Required Qualifications:

• Active state license as a Registered Physical Therapist.
• Current CPR certification.
• Health Certificate as required by state regulations or specific contracts.
