TITLE: Licensed Vocational Nurse (LVN)/Health Coordinator

DEPARTMENT: Head Start/Early Head Start and EHS Child Care Partnership

REPORTS TO: Health and Nutrition Program Coordinator/or Designee

FLSA STATUS: Non- Exempt, Full-time


Under general supervision, assists with the planning, coordinating, and implementing of the health services of the program. Collaborate with community organizations, content specialist, and local providers to ensure health services are provided to the children. Monitor that HS and EHS children are up to date on an age-appropriate schedule of well child care and provide expertise and guidance concerning the health and safety services. Ensures adherence to organization and program policies and procedures.

Essential Functions and Responsibilities

Include but not limited to the following;

• Review health status and follow-up: review, evaluate, and interpret health records and maintain a system for determination of health/dental status with appropriate follow-up.

• Monitor that children are up to date with their preventative and primary health care.

• Determine the need for medical and or dental follow-up and assist with obtaining further diagnostic testing, examination, and treatment, as needed.

• Meet with or conduct phone interviews with parents regarding health findings and assist families in understanding medical information and the need to complete prescribed treatment plans.

• Assist families in obtaining needed medical care by making referrals to appropriate community health care providers.

• Assist in training staff in hearing and vision screening and tracking the completion and results of screenings.

• Work with contracted Nurses to develop and implement individualized health plans (IHPs).

• Track and monitor children with health care plans and those that require medication administration at the sites.

• Provide assistance and information to Family Service Associates in their support and case management of families related to health services and health follow-up.

• Work collaboratively with Family Service Associates to review and discuss health reports, files, follow-up, and individual child health concerns.

• Conduct site visits to observe children with health concerns.

• Assist Health and Nutrition Program Coordinator in preparing and updating health policies and procedures, forms, reports, tracking systems, in accordance with LACOE GIM's, Head Start Performance Standards, Community Care Licensing and other regulations.

• Generate reports using various systems such as excel, ChildPlus, etc., timely and as requested.

• Provide health education trainings and workshops to parents.

• Provide trainings to staff.

• Attend meetings as required.

• Plans and facilitates health and safety inspections.

• Perform other job-related duties as required.


A. Education

1. Completion of licensed vocational nursing program from an accredited technical educational institution.

B. Experience

1. One year of nursing experience in a community setting. Pediatric or Head Start/Early Head Start experience preferred.

2. Experience working with parents, children, and health professionals.

3. Experience coordinating medical services with community health programs, nursing schools, pediatrics services, and other related health practitioners is highly desirable

4. A minimum of one year providing trainings.

C. Skills

1. Ability to deal with conceptual matter.

2. Ability to plan, organize, allocate and control substantial resources.

3. Demonstrate knowledge of correct English grammar, spelling and punctuation, ability to communicate effectively, both orally and in writing.

4. Excellent interpersonal and organizational skills.

5. Computer literate (MS Office Skills, Word, Excel and Power Point).

6. Regularly required to operate standard office equipment (computer, Tablets, iPads, fax, phones, printers, etc.).

7. Spanish/English fluency required.

D. License/Credential/Permit:

1. Current and valid CA license as a Licensed Vocational Nurse.

2. Red Cross Basic First Aide Certification and CPR Certificate are preferred.

Health Requirements

1. Must have verification of T.B. clearance at the time of employment issued within six (6) months prior to employment, to be renewed every four years.

2. Must pass pre-employment Health Screening, T.B. Test and Drug Screening.

3. Must submit to a flu vaccine once a year or submit a waiver.

4. Must provide vaccination records for influenza, pertussis and measles, at the time of employment.

5. COVID-19 vaccine or must apply for exemption.

Special Requirements

• Must possess a valid California driver's license, have use of a motor vehicle, and provide evidence of auto liability insurance as required by law. Good driving record.

• Must be 18 years of age to apply.

• Must possess fingerprints clearance by the Department of Justice (DOJ)/Federal Bureau of Investigation (FBI), Live Scan Agency or State Licensing Agency.

• Fingerprint clearance must be obtained for continued employment with MAOF. A clearance number revoked by DOJ/FBI or State Licensing Agency will result in immedi-ate termination of employment.

• Regular attendance, Monday thru Friday, ability to work regular schedule assigned.

• Must provide updated educational transcripts as required.
