Overview:

Medical Social Worker II- Continuum of Care

PRN/OCC, Hours Vary, Day Shift

Fridays and Saturdays. Occasional shifts may be available during the week.

Continuum of Care:

The Continuum of Care Department is staffed by Coordinator, RN Care Managers, and Masters Level Social Workers. They are involved in the assessment, planning, coordination, evaluation, and facilitation of services to meet the patient's and family's comprehensive health needs. Effective and open communication are necessary to promote quality and cost effective outcomes. A multidisciplinary, collaborative, team approach is essential in working to meet the patient/care giver transition of care needs and facilitating a safe discharge. Staff must be patient advocates and aware of community resources. Seasoned and newer employees work together in a challenging, ever changing environment. The department is staffed 7 days a week. The department works all minor holidays. Scheduling is centered on team and department needs.Case Managers and Social Workers are available for all patient care areas including the Childbirth Center, SBU, ED, and outpatient areas.

Position Summary:

Provides direct and indirect social work services to assigned patient care units including pre-hospital services, in-hospital service and discharge planning services. Assist patients and their families in seeking help with emotional, social, psychological and financial problems.

Responsibilities:
Conducts discharge planning meetings on assigned floors together with nursing and support services in order to locate patients in need of social work services/discharge planning.
Interviews patients and/or significant others to obtain pertinent background/social/financial data. Assists patient/family members in locating appropriate community services to meet their specific needs.
Interprets available programs and services to patient, family and patient's physician. Refers patients to social agencies to provide services required.
Uses casework method in counseling with patient and family as needed.
Cooperates with hospital staff and physicians and keeps them informed of progress with patients via verbal communication and chart notes in patient's medical record.
Maintains monthly statistical report on status of caseload.
Follows policies, procedures, and safety standards. Completes required education assignments annually. Works toward achieving goals and objectives, and participates in quality improvement initiatives as requested.
Perform other related duties as assigned or requested.
Qualifications:

Minimum Education: Bachelor's Degree from CSWE accredited baccalaureate social work program.

Minimum Experience: Two (2) years of experience.
