Qualifications:

RN, LPN, License
CPR Certification
Valid Driver's License with Auto Insurance
Previous experience working in Home Health
Experience working with EMR system: Home Care Home Base a Plus!
Excellent written and verbal skills
Understanding of Oasis -E, and timely documentation

Responsibilities:

Administering medication to patients.
Creating reports to track the patients' progress.
Working with other professionals to create treatment plans.
Cleaning and re-dressing wounds if needed.
Measuring vital signs.
Monitoring changes in behavior and progress.
