Responsibilities

The Nurse Practitioner will conduct annual risk assessments, either in home settings or via telehealth, adhering to the scope of practice as delegated by the Collaborative Physician.

Key Duties:

  • Perform comprehensive in-home health risk assessments to identify active and chronic health conditions as well as physical, mental, and social needs
  • Gather patient history, conduct examinations, and formulate diagnoses
  • Document patient visits according to NCQA standards
  • Measure and record vital signs as appropriate
  • Facilitate case management referrals and collaborate with primary care providers
  • Communicate effectively with patients, caregivers, agency nurses, other providers, and vendors for coordinated care
  • Always adhere to OSHA Universal Precautions while performing clinical duties
  • Maintain patient confidentiality at all times
  • Attend required meetings and participate in ongoing professional development

Qualifications

Required:

  • Active and unrestricted medical license
  • Current CPR Certification
  • Board eligible or board certified in family medicine
  • Strong Electronic Health Record (EHR) skills

Preferred:

  • Experience or training in geriatric care
  • Excellent teamwork skills and ability to work effectively with diverse groups

Physical Requirements:

  • Ability to adapt to varying weather and home environments
  • Full range of mobility, including handling/lifting patients and normal visual/auditory capabilities
  • Lifting/carrying up to 50 pounds regularly
  • Willingness to drive up to 150 miles daily in both urban and rural settings

Compensation: USD $85.00 - $145.00 Per Year
