Responsibilities

As a Nurse Practitioner, you will perform annual risk assessments in various settings, including residential visits and telehealth, adhering to the scope of practice under the guidance of a Collaborative Physician.

Essential Duties and Responsibilities

  • Conduct thorough in-home health assessments to identify active and chronic diseases, along with physical, mental, and social needs during each visit.
  • Carry out patient histories, examinations, and diagnoses.
  • Document patient visits in line with NCQA standards.
  • Measure vital signs as necessary.
  • Initiate case management referrals and communicate with the primary care physician as needed.
  • Collaborate with patients, caregivers, agency nurses, and other providers to ensure accurate diagnoses.
  • Execute all clinical duties while following OSHA Universal Precautions.
  • Maintain patient confidentiality at all times.
  • Attend mandatory meetings and participate in special committees as required.
  • Engage in professional development activities and maintain relevant licenses and affiliations.

Qualifications

Required Knowledge, Skills, and Experience:

  • Must possess an active, unrestricted medical license.
  • Current CPR Certification.
  • Board eligible or certified in family medicine.
  • Proficient in EHR systems.

Preferred Knowledge, Skills, and Experience:

  • Experience or training in geriatrics.
  • Ability to work effectively in a team and maintain strong relationships with patients and medical staff.

Physical Conditions of the Role:

  • Ability to adapt to various weather conditions and residential environments.
  • Capability to perform a full range of body motions, including lifting and handling patients.
  • Regular lifting/carrying of objects up to 50 pounds.
  • Ability to travel in a vehicle for up to 150 miles daily in diverse urban and rural settings, with a valid driver's license if needed.

Compensation:

Minimum Pay Rate: USD $85.00 per visit

Maximum Pay Rate: USD $145.00 per visit

Compensation packages are tailored to the individual, considering various factors such as skills, experience, and qualifications.
