Responsibilities
In accordance with our core values, the Primary Care Provider is accountable for delivering high-quality healthcare across Fast Pace Health's services, ensuring optimum patient satisfaction. This role involves providing preventative care as the initial point of contact and maintaining ongoing comprehensive care to enhance long-term patient health.
- Conduct thorough physical examinations and assessments for patients of all ages (children age 3 and older).
- Manage chronic care for conditions like hypertension, diabetes, and asthma.
- Administer annual wellness visits for both adults and children, including vaccinations and preventive screenings.
- Order and interpret diagnostic tests to evaluate patients' clinical issues and healthcare needs.
- Collaborate with physicians and other healthcare professionals to create comprehensive care plans.
- Prescribe medications and treatments, providing clear home-going instructions.
- Refer patients to specialists or other healthcare resources as required.
- Educate patients and their families on health maintenance and preventive care.
- Encourage patient engagement and health literacy to foster shared decision-making.
- Participate in quality improvement efforts and value-based care programs.
- Emphasize patient-centered care, continuity, and strong patient-provider relationships.
Essential Functions:
- Deliver quality care while meeting patient volume goals (average of +18 daily).
- Experience preferred as a Telehealth Clinician.
- Provide leadership and guidance to clinic staff (X-Ray Technologists, Nurses, Front Office Specialists).
- Review patients' medical history, symptoms, allergies, and current medications to provide accurate diagnoses.
- Engage with clinical leadership and management concerning patient care and resolution of concerns.
- Complete patient charts accurately and promptly.
- Capable of functioning efficiently in a dynamic environment.
- Stay updated with evidence-based medicine and recent medical advancements.
- Implement clinical protocols in line with CMO and Supervising Physicians' guidelines.
- Perform responsibilities according to established NP protocols.
- Analyze patients' histories, symptoms, and diagnostic information to determine accurate diagnoses.
- Provide excellent patient care through in-person evaluations, focusing on safety, accuracy, and patient-centered information.
- Adhere to Federal and State regulations, including HIPAA compliance.
- Complete all mandatory compliance training to enhance the quality of care.
- Exhibit teamwork, communication, empowerment, quality care, and friendliness.
- Recommend improvements within the clinic leadership to enhance patient experiences and care.
- Communicate consistently with the team to support exceptional patient care.
- Build and maintain trust and credibility with employees.
- Maintain a positive work atmosphere through cordial relations with all clients and colleagues.
- Complete physical, cognitive, and environmental requirements as specified.
- Know and comply with company operational standards.
- Promote confidentiality and respect within the employee and patient culture.
- Consult with patients through various communication channels.
- Assist with urgent care tasks as primary care schedule allows.
- Perform other duties as directed by management.
Qualifications
Education:
- Master's degree in Nursing (MSN) and/or Master's degree in Physician Assistant Studies (MMS).
Experience:
- 1 year of direct primary care experience as a Nurse Practitioner (NP) or Physician Assistant (PA).
- Proficient in comprehensive primary care, chronic care management, annual wellness visits, and preventive health measures.
- Familiarity with HEDIS quality metrics for preventative screenings, vaccinations, and chronic conditions.
- Experience performing pediatric exams and treating children aged 3 years and up is mandatory.
Licensing and Certification:
Current License and DEA must be active, in good standing, and verifiable. Primary Care Providers must be capable of treating all ages and meet credentialing requirements. DOT certification must be obtained within 120 days of employment and maintained throughout employment.
