Your role at BHSN:
As a program supervisor you will work collaboratively with BHSN team members/departments including but not limited to Program Coordinator, Director, Health Care Coordinator, Nursing, Crisis, and Facilities Department to help lead and support the community residence team and the residents of the Breakthrough II. All shifts are "stay awake" shifts.

What your day might look like:
Oversees the daily operations of the Community Residence
Assist in mentoring, on-boarding, training, supervising and monitoring community residence Support Specialists Levels I, II and interns
Work collaboratively and communicate effectively with team members to ensure resident needs are met including but not limited to coordination of mental health/substance use/primary care services, obtaining funding, transportation, linkage to other services, care coordination and discharge planning
Coordinate and participates in regularly scheduled treatment team meetings and effectively communicates outcomes to team members to ensure resident's needs are met
Provide restorative services and accurate documentation of services in a timely manner in accordance with Federal/State regulations and BHSN policies/procedures
Complete NYSCRI Adult Residential Assessment with participant and assist participant to develop an individualized service plan outlining participant's goals/objectives and what restorative services will be provided to assist the participant
Develops service plans following the eleven restorative services as required by Office of Mental Health regulations and documents progress
Observe the resident self-administration of medication according to established protocols
Documents all services in accordance with Federal/State regulations and agency policies/procedures
Assists residents with daily needs including but not limited to cleaning, cooking, transportation, linkage to other services and discharge planning
Your skills and qualifications:
Bachelor's degree in human services or medical field strongly preferred
Management experience preferred
3 years' experience with vulnerable population required
Experience working with individuals recovering from severe mental illness and/or substance use disorders strongly preferred
Experience with crisis resolution preferred
Ability to work a flexible schedule to accommodate the needs of the program
Valid NYS driver's license, acceptable to BHSN insurance carrier, for required local travel
CPR certification required within 60 days of employment
Required to complete 6 hours annually in BHSNU Credits, participate/complete BHSNU Cohort on "Leadership Development" in addition to courses/cohorts as assigned
