Registered Nurse - Wound and Ostomy Specialist

Join our dynamic Wound Ostomy team as a Wound Ostomy Continence (WOC) Nurse, where you will play a vital role in supporting patients with unique wound, ostomy, and continence care needs. In this capacity, you will provide hands-on clinical support and serve as an educator and consultant, ensuring optimal patient outcomes throughout their acute care journey.

Key Responsibilities:

  • Assessment: Gather important data regarding patients' health and care needs.
  • Diagnosis: Analyze data to identify actual or potential healthcare issues related to wound, ostomy, and continence needs.
  • Outcomes Identification: Develop individualized care plans with expected outcomes.
  • Contribute to improving healthcare efficiency and positive patient outcomes.
  • Collect and analyze data to monitor and enhance WOC nursing practice quality.
  • Collaborate with the interprofessional team on quality improvement initiatives.
  • Review policies and guidelines to elevate the standard of wound, ostomy, and continence care.
  • Assess resources available for achieving desired care outcomes.
  • Support organizational decisions regarding cost-effective care solutions.
  • Assist patients in finding appropriate services for their care needs.
  • Evaluate the impact of resource allocation on patient safety and desired outcomes.

Qualifications:

  • Graduation from an accredited WOC Nursing Education Program with a commitment to tri-certification within one year.
  • Bachelor's Degree in Nursing required; Master's Degree in Nursing preferred or currently pursuing one with completion planned within five years.
  • Possession of a valid NYS Registered Nurse license.
  • 3-5 years of nursing experience.
  • Excellent communication skills with healthcare teams and external agencies.
  • Ability to foster a collaborative and collegial environment.
  • Clear articulation of expectations with patients and care team members.
  • Accountability for clinical decision-making and actions.
  • Adept at problem-solving and data analysis for effective solutions.
  • Commitment to continuous education and professional development.
