Location: Griffin, GA

Salary: $13.46 - $15.38 per hour, depending on program hired for

Schedule: Full-time, temp to perm

Start Date: As little as 2 weeks from acceptance of offer

Job Requirements:

Valid driver's license
Experience working with individuals with Mental Health and/or Substance Abuse diagnoses (preferred)
Fingerprint-based background check of the Georgia and Federal Criminal History Record
Drug screen
Motor Vehicle Report (MVR) for seven years
Bachelor's degree in a social service-related field from an accredited college or university AND One year of experience in a social service-related field OR
High school diploma or GED AND Three years of experience in a social service-related position OR
One year of experience at the lower level Social Svcs Tech 2 (SST011) or position equivalent
Must be able to lift and carry up to 25 lbs

Job Description:

Under broad supervision, the Social Service Tech provides a broad range of social services to patients/clients/consumers and their families. Key responsibilities include:

Conducting patient/client/consumer intakes
Participating in treatment team planning and performing crisis intervention
Assisting in dealing with personal and social problems
Providing supportive counseling to consumers and families and/or serving as a liaison for social services
Performing case management duties
Performing lead worker functions

Essential Duties and Responsibilities:

Working closely with clients to teach new skills for daily living and linking clients with available resources in the community for clients to live a more productive and independent life
Coordinating and providing PSRI/CSI/CM/ADSS outreach to a close load
Responsible for a caseload of 30-50 individuals to be seen face-to-face a minimum of twice per month or document attempts or explanation as to why no attempt was made as evidence by chart review
Ensuring treatment plans are in place, services are authorized, and the billable hours assure the agency will get paid
Responsible for tracking the dates that authorizations/BHAs are due, being mindful of the units per authorizations, schedule appointments for individuals' updates, and assisting clinicians if needed by supplying information for update paperwork or treatment goals
Monitoring MD visits to assure they occur at least once every 6 months
Documenting all face-to-face, collaterals, phone calls, no-shows, and attempts using a progress note or non-billable/contact note, and keeping events and confirming by the end of the session
Coordinating level of care changes and demographic changes, and seeking consultation and supervision on cases requiring multiple hospitalization, legal issues, or ongoing crisis
Providing support staff with any demographic changes and checking EHR regularly to assure individuals are keeping MD and Nursing appointments and release of information are current
Assisting individuals to gain access to necessary services
Participating in Center staff meetings/treatment team meetings and advocating for PSR-I services especially when slots are available, and attempting contact within one business day when notified of PSR-I individual's hospital discharges
Working with assigned productivity goals, ensuring appropriate use of resources and time by remaining fiscally responsible, and meeting the productivity requirements of 55% per month
Performing related duties as assigned by supervisor and maintaining compliance with all company policies and procedures.
