Speech Language Pathologist

The Speech Language Pathologist primarily responsible for continuous review of all aspects of his/her caseload to include: appropriate utilization of services; ensuring continued skilled need; monitoring of homebound status; review of documentation in the medical record; maintenance of interdisciplinary communication and discharge planning.

Job Functions:

Develops a care plan, which establishes goals based on nursing diagnosis.
Utilize med/surg skills such as phlebotomy, wound care, and teaching for chronic diseases.
Supervise, teach and provide clinical direction to LPN and CNA's.

Position: PRN

SLP Rates:

OASIS SOC - $135 per visit
Assessment - $105 per visit
Routine Visit - $95 per visit
Requirements:
Valid Virginia Speech Language Pathologist license
Prior experience in home health is preferred
