About the Role:

This is a fully virtual role that offers the flexibility to work from home while participating in essential in-person training sessions. This position fosters productivity, promotes work-life integration, and supports skill development through face-to-face onboarding.

Your work schedule will be Monday to Friday, 8:00 am to 5:00 pm (EST or CST), with occasional on-call responsibilities.

Your Impact:

  • Provide primary and acute care to patients through telephone and tele video modalities, focusing on those receiving home and community-based services.
  • Develop tailored clinical care plans for adults with chronic and complex conditions.
  • Accurately gather patient history, conduct physical exams, and implement appropriate treatment plans.
  • Identify and address gaps in patient care.
  • Work collaboratively with an inter-disciplinary team to meet the physical and psychosocial needs of patients and their families.
  • Educate patients and families on medication management, illness progression, lifestyle changes, and prevention strategies.
  • Coordinate care with other clinical team members and providers to ensure optimal patient outcomes.
  • Maintain comprehensive medical records and documentation in line with state regulations and company policies.
  • Participate in continuing education to meet state and certifying body requirements.
  • Prescribe medication within the limits of your state authority.

Minimum Requirements:

  • Master's degree in Nursing.
  • Active national Nurse Practitioner certification.
  • Unrestricted Family or Adult Nurse Practitioner license in Indiana.
  • Valid RN Compact license.
  • Minimum of 2 years' experience in managing complex care cases.
  • Experience with Electronic Medical Records (EMR) is essential.

Preferred Qualifications:

  • Possession of DEA registration or eligibility.
  • Active Medicaid number in Indiana.
  • Experience with care management for developmental disabilities and chronic illness.
