Are you a passionate and experienced Nurse Practitioner? We invite you to apply if you possess an active, unrestricted Family or Adult Nurse Practitioner license in Indiana along with a valid RN Compact license.
About the Role:
This is a fully virtual role that offers the flexibility to work from home while participating in essential in-person training sessions. This position fosters productivity, promotes work-life integration, and supports skill development through face-to-face onboarding.
Your work schedule will be Monday to Friday, 8:00 am to 5:00 pm (EST or CST), with occasional on-call responsibilities.
Your Impact:
- Provide primary and acute care to patients through telephone and tele video modalities, focusing on those receiving home and community-based services.
- Develop tailored clinical care plans for adults with chronic and complex conditions.
- Accurately gather patient history, conduct physical exams, and implement appropriate treatment plans.
- Identify and address gaps in patient care.
- Work collaboratively with an inter-disciplinary team to meet the physical and psychosocial needs of patients and their families.
- Educate patients and families on medication management, illness progression, lifestyle changes, and prevention strategies.
- Coordinate care with other clinical team members and providers to ensure optimal patient outcomes.
- Maintain comprehensive medical records and documentation in line with state regulations and company policies.
- Participate in continuing education to meet state and certifying body requirements.
- Prescribe medication within the limits of your state authority.
Minimum Requirements:
- Master's degree in Nursing.
- Active national Nurse Practitioner certification.
- Unrestricted Family or Adult Nurse Practitioner license in Indiana.
- Valid RN Compact license.
- Minimum of 2 years' experience in managing complex care cases.
- Experience with Electronic Medical Records (EMR) is essential.
Preferred Qualifications:
- Possession of DEA registration or eligibility.
- Active Medicaid number in Indiana.
- Experience with care management for developmental disabilities and chronic illness.
