JOB OVERVIEW This is a home-based primary care program that provides health care services in the comfort and convenience of the patient s home. It s like an old-fashioned house-call for patients who are eligible to join. The program provides primary medical care, social support, and help with nutrition and medications.
The Field Based Physician will provide high quality, evidence-based, in-home patient care to complex and high-risk patients.
Primary goals and objectives: providing best-in-class patient care, reducing avoidable hospitalizations, decreasing unnecessary emergency department utilization, and increasing accurate risk adjustment factor scores.
Our program is designed to provide In-Home patient care, SNF Visits, Assisted Living and Virtual Care Visits to our most complex patients living with multiple chronic conditions. The Physician will see approx. 4-12 patients per day. The Field Based Rounding Physician will be the anchor to the assigned Care Team and will work in conjunction with the Primary Care Physician and Specialists to ensure and align with the Quadruple Aim: the patient receives the right care, at the right time, in the right setting and at the right cost.
REQUIREMENTS
- Medical Degree
- Board Certified Internal Medicine, Family Practice, or Emergency Medicine
- Will consider New Grads
- Active, non- restricted California Medical License
- ACLS Certification preferred
- Minimum of one to two (1-2) years of experience in urgent care/ outpatient clinic setting, experience in home-based care preferred
- Experience with Geriatric population
